How To Join the
Practice
Until you join (register) with the practice we can only
offer you urgent or emergency medical services if you
are living temporarily with a patient of our practice.
To join (register) with the practice you must live
within our practice area (for clarification ask at
reception). We take full families only.
We try
to keep the registration process as simple as possible,
you do not need an appointment, just come to reception
at the surgery. There are some things to do so that we
can get your medical notes from your previous doctor.
 |
A
form needs to be completed for each family
member and handed in to reception.
|
 |
All new patients will be asked to see the
practice nurse, within a month of registering,
for a simple health check. This enables us to
obtain details of your past medical problems and
to provide better care. |
When registering with the practice each patient has
the right to express a preference of practitioner, this
can be indicated on the registration form, and whilst
the practice will endeavour to comply it may not always
be possible and if this were to be the case, an
explanation would be offered.
Disabled Patients
There is full wheelchair access at the surgeries and
also a disabled toilet.
